Getting Approval for Dental Treatment Through The Care Credit Extended Loan Program

Here are a few easy steps:

1. Go to www.carecredit.com and click the patients/clients button located in the top of the left corner.

2. Click the "APPLY NOW" button located at the top of the screen.

3. In the "Search For My Doctor" box located in the main text enter the phone number 9377489240. (Do not put dashes or periods within the phone number.) OR type in the name Meclosky in the same search box.

4. After you confirm this practice, click the "APPLY NOW" button.

5. The next page is the form for Consent to Electronic Communications. Go to the bottom of the page and click the "Accept and Continue" button.

6. The next page is the actual form for the approval for financial Aid. YOU MUST FILL OUT ALL FIELDS THAT APPLY TO YOU! Then click the "Agree and Submit" button.

You have now completed the process for credit approval. Once you have clicked the Agree and Submit button you will be taken to the next screen to see if you have been approved. If you have been approved, the screen will show you your available credit line in the top right corner.

You are now set to start dental treatment!
